Hello & a quick survey

Hi guys, long time no, um, forum-ing I guess

Looks like the forum is as lively as ever. I've missed out on heaps!

Uni is going good. Lots of work to be done though. Speaking of uni, I have one teeny tiny favour I would like ask you guys... I have a group project for one of my subjects, Science Communication, which is due soon. One of the project aims is to gather public opinions on our topic (ours is global warming by the way) in the form of a survey.

I would greatly appreciate it if you guys could spend five minutes completing our survey (it's only three pages long...not many questions...) Here is the link for the survey (it is a PDF file):

There are interactive fields so you can just click on your answers and type things in etc. I know how much people hate filling out surveys but this one is for a good cause...I think... Please send your completed surveys to the email address listed at the end of the survey. Thank you!!!

Thanks a lot for that. For some reason, your file seems to be corrupted! When I opened your file up, I couldn't see any answers and the whole survey looked wrong (lots of bits missing) Maybe it was the way I compressed the file. It did something to it...
I will see if it happens to others.

Thanks phil. I got your file. It gives me the same message that scott's one gave me and same goes for another friend i got to test it out.
I don't what is wrong. I'm using version 7 of acrobat. What version are you guys using?
I may have to re-upload the file. This time, I will put the unreduced version up.

I've just put a new file up. So hopefully, it will work for the others this time. I don't expect you guys that have already done it to go through it again
Can I just ask also whether you guys (Phil and scott) saved/downloaded the file first and then filled it in, resaved it and then sent it? Or did you fill the survey in first via the weblink and then saved it?

I opened it, filled it out and saved it then sent it...I'll try again...adobe version 6 reader...I can't make changes and then resave...do you have it in word?

thanks scott, thanks phil i do have the file in word though but there are no interactive fields (if there is a way, I don't know how to do it...)
well here is the link for the word file:

see how that goes. for the ticks, just use any character to show your preference and for the circling bit, just change the colour of the text and bold it. you guys have the license to be creative (except for changing the survey questions themselves of course )

ok, i have now uploaded a new word document with form fields in them (yay! I figured it out!). This one is not as fancy as the PDF and I couldn't figure out how to let only one choice be selected (for the questions that required only one answer)
Anyway, for those of you who have acrobat professional version 7, try using the PDF file. Otherwise, please use the word document. (The links posted above are still valid. it should take you to the newest version).

Thanks for all the responses so far. You guys are a great bunch. I knew how much you all secretly love to fill out surveys

Hey Mike, if I did know of any, I'm staying well away from all that! It's funny though, I used to print all those lecture notes and other materials and now I'm the one having to pay for them all...it used to be so good when I could just grab one hot off the presses corruption at its best...
