Its nothing to do with the refresh rate and everything to do with the connection type you specify. Are you selecting DVI output and using a DVI cable?

Yes I was, DVI match DVI on PC and monitor (settings). There's reports of others on the net having the same problem and solution.

Of course now I'm not using DVI, I'm using the standard old style plug and settings on both (how I got it working). Both the monitor and PC have always correctly identified DVI vs VGA plugs by themselves.
