I am no expert but with my GEM mount the three main consistancy factors seem to be:
1. A level scope.
2. Alignment of the OTA with the mount axis so they both point at the same thing.
3. Even the slightest play in the worm gears or collette will cause the scope to rotate slightly out of the original alignment when the balance shifts as you slew to new objects, (especially when you cross the meridian). This will also stuff up your polar alignment.
I don't know if any of these are applicable to your set up but it might help.
