The current (2.2.2g) Drift Alignment Help text makes no mention of changing the dec Guiding mode from auto to off, following a calibration.

Does this still apply?

While the following is noted as being incrporated into 2.2.2f "Do not display polar align error on graph unless dec guide mode is set to Off", I have found it can be folly to make what might seem to be common sense assumptions with software!

You have to turn dec guiding off to do the drift align. This was covered recently in the stark-labs-astronomy-group mailing list. See also http://openphdguiding.org/man/Tools.htm#Drift_Align

As such, do we also only watch the dec trend line for both az and alt?

I haven't used the Rev G OpenPHD, but just a couple of versions earlier (maybe D?) the Drift Align Tool did the turning off of DEC guiding for you. Pretty sure. Once the tool was closed, the DEC guiding goes back on for you.
