Graham,
What guide cam are you using? What driver version?
What mount connection? (ST4? ASCOM? EQMOD?)
If not ST4 what mount driver version?
I've done testing of the latest PHD2 with a couple of guide cams.

I've installed PHD2 okay with Windows 8.1 (last week, in fact). No problem with the C++ distribution libraries - it just updated them.

The prerelease program (http://adgsoftware.com/phd2/snapshots.html) has reached 2.2.2g. This now supports bad pixel maps, auto exposure, and improved auto star-find. Chinese language too if you are interested.
