I think John Hurt will be Doctor "8.5", the incarnation between McGann and Eccleston. When he speaks to the doctor, he is talking in his past tense, and the current Doctor may not know who he is if it is a future Doctor.
Anyway, was a great episode with a cool ending. I hope it goes on well in the 50th Anniversary Special and not fizzle out like so many Steven Moffat episodes do. They start so well and get wrapped up in the last 10 minutes instead of being continued next episode.
I also do not mind Matt Smith at all, he is quirky enough to be a good Doctor, I think the scripts really let him down sometimes.
