Arthur Clarke had the answer in one of his novels in the Rama series (Rama II I think.) The answer was in providing the nanobots with a coating that adapted itself to the environment it was in - glib but amusing - and also self-destructed if it malfunctioned.
That could make gallstones look quite mild.
