I've been threatening myself to start taking proper notes and logs for a while, now with some suitable forms I might actually making good on my threat!
I've also seen some nice forms from Jeremy Perez on the CN forums..

great observing report style and some nice sketches.. in lieu of DSO imaging equipment I might have to start sketching some myself .. although my artwork in high school was always more cartoony so I don't know how well that will translate
