"some what random" makes it a little harder.. hmm.
Can you replicate it at all on demand? Does it not work, then work? Or vice versa?
Or is it some threads and not others?
You could try turning off the Enhanced Image Browser in your User Options:

Down the bottom.
Otherwise, most problems like this are due to:
- Javascript blocking
- Popup blocking
- Firewall blocking
Make sure all of those are set properly too.
