Hi Yugant,
You just need the QHY5 driver for windows and PHD, nothing else to get yourself guiding.
ASCOM is only needed if your guide program doesnt directly support the QHY5
You will need a RJ12 cable to connect the QHY5 guide port to your NEQ6 guide port as well.
Install driver, plug camera in, (actually i think it is the other way around), start PHD, select star, press guide.
