Hi Steve - perhaps this is a quality control issue with some of the mounts slipping through with less than satisfactory workmanship: I have no problems with my mount and the specific issues you've raised, it is all very snug without play/slop etc (and I'm a pedant for these sorts of things.....)
The plastic milk container washer in the tripod head well is good for alleviating friction between it and the mount base - I was lucky enough to find a plastic washer of the exact dimensions to save hacking into milk containers there: people should be aware that when adjusting azimuth of these mounts that they need to have the central bolt slightly loosened, not enough to introduce any play that could intefere with alignment adjustment, but sufficient to allow the mount to rotate on the tripod head. Then, of course, it is "pinched up" to prevent movement.
Personally I am about to commission my home-made pier, the tripod will then go into hibernation except for "on the road" useage.
