I had a similar problem when I switched plans 512 to1500 broadband.
Even though all looked well(ADSL and PPP lights on) and procedure was followed modem would not connect. Drove me mad. Contacted ISP(TPG), went through their diagnostic protocol - turned out there was a fault on my line which the Telstra engineer fixed at his leisure!!!!!
Perhaps that's your problem too.
