If your using EQMOD then Ascom pulse guiding is the PHD2 recommended way of guiding and not ST4 method ( refer my item 11 )
Ascom pulse guiding with eqmod requires -
EQ direct cable ( shoestring cable ) from laptop direct to mounts handcontroller port
USB2 or USB3 cable from guide camera direct to laptop
