Possible problem with EQ6-R Mount

Currently imaging M68 with 2 minute guided dithered subs
Everything going great, went inside to grab a coffee came back out 10 minutes later and found mount in almost the home position ( slightly off centre ) but DEC and RA setting rings show “0” where they are suppose be
PHD2 obviously went haywire with a warning sign “lost pulse “
BYEOS still taking subs which are squiggly lines

Unparked scope and slewed to M68 again with EQMOD ended up spot on centre
Guiding working ok again and now taking subs again , all ok

Question: as the home position is not exactly where it was when I first started the session, could the RA clutch or gears / belts on my EQ6-R have slipped a bit during the capture session ??

Everything on the mount is rock tight , as I check it every time

Any advice on probable causes appreciated
