Meade Help Required - Collimation

After resolving my focuser problem, I now have a collimation problem.
The scope is a 14" Meade LX200 ACF.
When I look down the tube, I see the attached imaged. Something is does not look right. (See the area around near the red mark). It looks like the primary mirror is not square. Any suggestions on what is wrong? Can this fixed by adjusting the secondary?

When I get what is close to focus I get a very poor star image, a very noticeably fan shape image, not a round dot.

On the Meade which set of screws adjust the collimation, the inner or outer set. See the second image.
