ok, i have now uploaded a new word document with form fields in them (yay! I figured it out!). This one is not as fancy as the PDF and I couldn't figure out how to let only one choice be selected (for the questions that required only one answer)
Anyway, for those of you who have
acrobat professional version 7, try using the PDF file. Otherwise, please use the word document. (The links posted above are still valid. it should take you to the newest version).
