The first thing that I would check is balance in both RA and Dec axis. You can see those symptoms if the mount is too heavy in either axis. I noticed that my CGEM seems more susceptible to that than my previous CG5.
You can also get similar symptoms if the mount is perfectly balanced in RA. It needs to be slightly biased in one direction or the other to keep the worm constantly engaged.
