EQ8 Pier Plate Template

Hi,
Does anybody know where I can find plans to make a plate that sits on top of a pier for an EQ8 please.
I thought I had one on line somewhere but I'm struggling to find it again, probably because there are so many manufactured ones out there.

I can't find the original that I got this information from but this is the drawing for the one that I haven't around to having made yet.
