TOAG Canon bayonet adapter

As the title suggest I'm trying to track down a specific Orion Thin Off Axis Guider (TOAG) Canon bayonet adapter that effectively turns the TOAG into a T-ring and enables its use with a coma corrector without messing up the distances involved. The TOAG was generously gifted to me so now need to find the extra bits to make it work :-) Any help greatly appreciated.
