What do these things mean?

What is the difference between pixel dimensions and document size?

Is the document size the PHYSICAL size that the image will print at?

I want my image to print at roughly 60 x 40 cm. Are the settings I have used in the screenshot correct?

The number of pixels x and y is just that, nothing to do with printed size.

The number of pixels/inch determines document size (assuming you are printing 1:1 ratio). In this case, the settings are correct.

How big can I print a photograph out of a 5D MKII and still keep good resolution?

I have to keep the converted JPEG DPI (PPI) at 200

The figure quoted seems to be to print at about 300 pixels per inch for high quality, 150 ppi for OK quality.
