I had a similar thing recently with my EQ6, dropping up to 40" at seemingly random intervals, also using PHD. When mine did it, it would sound like the mount stopped tracking for about a second before kicking back in. I didn't get to the bottom of it because I decided I'd have enough with the EQ6 and bought a new mount. I'll eventually get around to fixing it, whether to sell or to keep as a backup, so I'll be watching this thread closely.

I'd assumed my issues might have been electrical in origin. I checked the voltage output of my battery which was fine, watched the power light on the EQ6 while it stopped tracking and saw no flash/dim of the light.

I don't have the AZ-EQ6 so I'm not sure about the bolts. Top three could be to hold the motor's frame, bottom two could be to hold the worm casing...
