Probably some gumby didn't assemble it correctly. I just loosened the motor nuts and pulled it against the belt and tightened again.

I could see when I had the worm gear end float cover off that when I hit alternate buttons on the controller that the worm wasn't turning. That could only be a lose belt or a lose gear on the worm shaft. Fortunately it looks like it was the former.
