Precise Parts are by no means cheap but it is so easy to get stuff made. Plus, compared to the outlay that you put in for mount, telescope and camera it really isn't that much more.
Just make your way through the drop down menus and put in the distance.
As this is what they do, you know it'll be good and if it isn't perfect they'll machine and send you out another one.
